Full Mouth Dental Implant Reconstruction -Audio Explanation With Burbank's Ramsey Amin, DDS

Full mouth dentistry with dental implants and crowns can get very complex.  I treated this patient in my Burbank office with nine dental implants and a total of more than 20 crowns. The full mouth reconstruction involved the upper and lower teeth and opening the bite to be taller.

Thin Receding Gums And Dental Implants – What's Important? Video by Ramsey Amin, DDS

Thin gums area major cause of gum recession and can cause problems with dental implants too.  You are genetically given the thickness of your gums.  When it comes to dental implants…the thicker the gums the better.  This is also true for natural teeth.  This video explains some of the important issues.

When someone has receding or thin gums, I can surgically correct this by a gum graft. So if the gum is too thin, it can be thickened.  Here is a photo of a gum graft I did to cover the root of this long tooth.

 

There are a couple of main issues associated with thin, fragile gums and dental implants.

1. The implant shows through the gums as a grey or grayish tint. This does not look good.
2. The gums can recede causing the bone to recede too.  If this happens, the threads of the implant can show. This becomes a long term issue.

Solutions I use: (much more detail in the video)

1. Gum and/or bone grafting before or during the implant procedure to thicken the gums
2. Use of a custom abutment made of zirconia or titanium
3. Proper implant design
4. Proper crown design
5. Precise placement of the implant to begin
6. At the time that I place implants, if possible, I move some of your existing gum from the palate side to the outside of your implant.
7. Platform switching design

As you can see, there is A LOT to dental implant success.  It is not just a screw put in the jaw!  Many dentists don’t bother with the gums and just put the implant in.  That is fine for the short term, but in time, you will probably develop problems.

A patient in my Burbank office asked me about the long-term with implants. Dental implant complications can occur.  Most focus on the surgery, but what I am talking about is what happens later down the road. Later, meaning after the crown, bridge or full implant restoration has been attached to the implants.

This video reviews two problems that can occur with a dental implant months or years down the road.

Keep in mind that "regular dentistry" has complications too.  Fillings break, crowns wear out, root canals become lose their seal, etc...unfortunately nothing is forever.  But with good upkeep and regular visits, potential problems can be taken care of more easily than waiting. The skill and training of your dentist plays a huge role.  Dont choose your dentist by price alone.

Immediate Dental Implant Video by Burbank's Best Dentist 2010

Immediate dental implants are amazing.  Your tooth can be replaced in about an hour. This patient found me from the Burbank Leader Newspaper's "Best of Burbank" issue (Best Dentist).

He had a Maryland bridge for his front tooth that kept falling out.  It was also very unattractive to him.

Using intravenous sedation, a dental implant and a temporary crown was done.  I purposely keep the edge of the temp shorter for the first three months after the surgery to prevent overloading the bone.  That can cause the implant to fail.

Immediate dental implants can also replace all of your teeth at one appointment or perhaps just a section of missing or bad teeth.  In the past, this used to take a year or so to complete. 

After 3 months with the temp.  I will remove the temporary crown and place the final porcelain crown.

Bone Grafts...How Long Do They Take to Heal? Dentist in Burbank Video

Some bone grafts for dental implants take longer to heal than others. In this video, I cover several factors that determine how long your bone graft takes to heal.  Remember that many times the bone grafts and dental implants can be done simultaneously.

 

 Factors

• Number of walls missing
• Upper or lower jaw
• Type of bone material
• Membrane type
• Block or particulate bone
• Rebuilding height, width or both 
• Concurrent gum graft

Most bone grafts are ready for dental implants 4 months after the surgery. Some will need 7 - 9 months.

Apicoectomy Failure - Burbank Dental Implants Success Video Testimonial

Apicoectomy root canal procedures can and do fail leaving a bone defect. This bone defect can become problematic when having a dental implant. Even though a root canal and an apicoectomy can "save" a tooth, sometimes they are still lost.

3 Ways To Avoid a Sinus Lift Bone Graft for Dental Implants - Burbank Dentist Explains

Streamlining my patient's dental implants by reducing time, number of visits and cost are important to me.

The sinus lift bone graft is an extremely common procedure that I do in my office.  In fact, I am one of the few in the world to be trained by the inventor of this procedure.  Hilt Tatum first performed this procedure 36 years ago!

The procedure is exclusively used to build bone height in the back part of the upper jaw for dental implants.  When teeth are lost, the sinus bone expands making the bone small for implants of adequate length.

Although the sinus bone graft is common and is necessary in many situations, this video describes three ways to possibly avoid it all together!  These really do work!

Chris had a very unique situation.  He had an unusual dental implant placed 10 years ago that needed to be replaced. A sinus bone graft was necessary in order for me to not penetrate the sinus that sits above the upper back teeth

Here was his situation in pictures.  The original implant had lost almost all of its bone. It was a bit large and placed in an unusual manner.  His sinus which was right above the implant, was touching the implant.

 
I removed the implant and did a sinus bone graft to make sure we had plenty of bone to anchor the new implant. I also want to try to correct the bone loss on the tooth behind the implant. Note the tremendous gain in bone height!

 

The implant was placed using IV sedation for all of the procedures:

I made him a new tooth on the dental implant that now fits the way it should.

Dental Implant Overdenture Locator Attachments...Patient Information

If you already have dentures, dental implants can be used to "lock" your denture to the bone. This is called an overdenture.

Please see this post for some overdenture basic information.

There are many choices for securing your implants to the denture. You should know that are bars or individual anchors. I will focus on the locator attachment.

Dental Implant CT Scan Video

The power of interactive CT cone beam scans in implant dentistry is amazing!

It allows me to plan the surgery in advance.  I can see things that I could never see with traditional x-rays. This man from Glendale, California was missing all of his teeth and wore regular dentures.

The CT (CAT) scan allowed the patient and I to really appreciate the bone loss he had suffered due to missing his teeth for so long. A cone beam CT scan exposes you to minimal radiation and maximizes safety.

Four dental implants were placed and an overdenture made for the bottom jaw.  He has been thrilled. Even with his bone as thin as it looks, a bone graft was NOT needed due to planning with a CT scan. He said the bad breath he use to have is much better since the denture does not lift anymore.

Video - How Long Do Dental Implants Take to Heal?

Dental implants can require some time to heal before they become integrated to your jawbone.  Teeth can be made on the implants on the same day that the implant is placed, or a few months later. It is quite variable.  Having a bone graft may or may not prolong that time.

Video Essay of A Dental Implant Patient

This short video depicts a single man in his 50's who I treated. 

He was missing a lot of back teeth and was embarrassed that he used removable dentures.  He told me he had to go to the restroom when dining out, to take his teeth out and wash them in the sink.  Food would get stuck under the partials and made him very self-conscious.

I placed six dental implants and a sinus lift bone graft.  He has been thrilled...and never again does he have to remove his teeth!

Video - Short, Narrow Dental Implants - Do They Work?

Narrow diameter and short dental implants DO work.  I have been using them for the last 10 years! In some cases you can avoid bone grafting by using them.

 

They just need to be used with caution, especially if you have a strong bite.  Short and or narrow implants are best for the front of the mouth where the bite force is less. They will work better for you in the lower jaw than in the upper because the lower jaw bone is more dense than the upper.  

 

In this post I am not talking about "mini dental implants."
